Great question! Starting an NEMT business takes more than just buying a van — here's what actually matters from experience:
Get Licensed & Compliant First
You need to be Texas Licensed, fully insured, HIPAA compliant, and MCO/Medicaid certified. Without these, you can't bill insurance or work with healthcare facilities.
Know Your Service Types
NEMT isn't one-size-fits-all. You'll need to decide if you're doing ambulatory, wheelchair, stretcher, dialysis, mental health, or IDD transport — each requires different vehicles and driver training.
Build Healthcare Relationships
Your real clients are dialysis centers, hospitals, rehab facilities and care coordinators — not just individual patients. Partner with them early.
Get Listed on NEMT Directories
Platforms like MedicalRide.org help patients and facilities find you. Get verified there as early as possible.
Invest in Technology
GPS tracking, online booking, and HIPAA-compliant scheduling are now expected — not optional.
